Information for service users

What is the Outcomes Star?

The Outcomes Star is a tool to help you and your keyworker explore what support you need, where you are doing well, and where you may want to make changes to have a better quality of life.

There is an Outcomes Star: User Guide that explains about the Outcomes Star and how it is used and this can be freely accessed from this website.

Before you complete your first Star Chart, your keyworker will also talk you through the Outcomes Star, the two important parts of the Star - the 'journey of change' and the ten ladders. They will also explain how the Star links to your support and action plans. 

What is the Outcomes Star System?

If you already complete Outcomes Star charts with your keyworker, the system won't be a big surprise to you. It is an online version of the same form, with the added bonus that your Star Chart is automatically created on-screen once you've completed the form.

You can compare your latest chart with your first or previous versions. And this means you can quickly see where you have made progress and what areas you need to work on with your keyworker.

Is my personal information safe?

It is completely understandable that you would be concerned that your personal information is kept private. For this reason, the system is password protected and does not store very much information about you at all.

In developing the system we have done everything we can to ensure it is that it is impossible to associate the information on the system with a real person unless you need to know - for instance, your keyworker and you, your keyworker's manager and their manager.

Keeping the information anonymous

Your keyworker will give you an anonymous identifier - some letters and numbers - which will be kept on your client file, in a locked cabinet. This will identify you on the system and will link you with your Star Charts.

Limiting the amount of information stored on the system

The only other personal information kept on the system, besides your Star Chart results, is your date of birth, the name of your keyworker and some information that helps your organisation ensure they are providing services in a fair way, such as race and ethnicity.

Your name is not kept anywhere on the system.

Who can see my information?

Only your keyworker, their manager and the chief executive of the organisation can access this information about you. Other people who use the Outcomes Star System do not have permission to see any information on you.

No other service users, keyworkers and managers can see your Star Charts or your date of birth.

How will my information be used?

Firstly, the information is used by you and your keyworker to see where you need most support.

The Star Charts you submit are shared anonymously with the whole organisation. This is for good reason - to allow the management to see where the work their staff do is going well, and where they may need to invest more time, training or money to improve things for service users.

Do I have to have my Star Charts online?

No. Before your Star Charts are added to the Outcomes Star System, a worker will ask you for your consent. If you don't understand, or have any concerns you can raise them at this stage. And you are not obliged to take part - however, those service users that use the Star generally find it helps them, with support, to make great changes in their lives at a pace that feels good for them.
