Supported housing and hostels
The following is a list of all the information each service provided during the five months of the
Lewisham, Southwark and Lambeth pilot, arranged within the five
Communities and Local Government outcome headings. Definitions of the indicators and an excel spreadsheet were provided to the eight providers taking part in the pilot. Each of the pilot providers had (or introduced) their own internal distance travelled tools (e.g. the Outcomes Star) and some of the indicators within the LSL initiative drew on information gathered using those tools to give an overall count of numbers of clients progressing in certain areas of their lives.
1) Economic well-being (5)
- Established/maintained claim for housing and other key benefits
- Engaged with a plan to reduce rent arrears/ debts
- In paid employment
- Less than eight weeks rent arrears
- Improved money management/budgeting
(number showing improvement on money scale of internal distance travelled tool).
2) Enjoy and achieve (5)
- In training, education
- Improved use of time, including hobbies/leisure activities
(from use of time scale of internal distance travelled tool)
- Improved work skills (in work-related training – volunteer, apprentice etc.)
- Improved employability

- Social networks/more positive relationships
(improved on social networks/relationships scale of internal tool).
3) Health (9)
- Register with GP
- Engagement in treatment plan:
- number of clients in need of a treatment plan for physical or mental health and, of them, number with a plan and engaging with it
- Reduce use of Accident and Emergency:
- number of clients who used A&E in the six months prior to coming to the service and average number of times each;
- number who used A&E in the six months of the pilot and average number of times each.
- Reduced emergency psychiatric admissions: as above
- Sustain medication:
- Number of clients with prescribed medication for on-going physical or mental health issues
- Of those, number of SU complying with daily medication with support, on monthly injection, self-medicating and refusing medication
- Improving responsibility for physical health

- Improved mental well-being

4) Stay safe (7)
- Planned move-on within timeframe agreed with provider and where:
- Number leaving the service within the pilot period
- Number with planned move-on within the agreed time period
- Number with planned move-on outside the agreed time period
- Number with unplanned move-on
- Number to private rental, social housing or other project
- Move-on bids made:
- Number considered ready for move-on
- Number for who move-on bids have been made and to whom (use menu of SP options)
- Average number of bids per SU Case closure
- Tenancy sustained in hostel
- Number who kept the terms of the tenancy
- Reduced high-risk activities or situations
- Number with initial risk assessments that indicate high risk
- Of them, number with lower risk rating at review
- Move-on situation sustained for six months, one year
5) Make a positive contribution (1)
- Engage with plan around offending behaviour (number showing improvement on offending scale of internal tool)
