How many reviews go on each Star chart?
We recommend that key workers start to use a new Star chart after a maximum of four reviews. Less if you have recorded separate scores for you and the service user as the chart will start looking messy and complicated.
If you want to see how someone has changed since they first came to the service, you could copy the reading at first assessment on to new Star charts. Perhaps discuss this option with service users and decide between you.
